Do I need to remove the door panel to install this?

Access is from the interior side of the door. You'll need to remove the inner door panel to disconnect the old mirror and route the harness for the new assembly. Basic hand tools and a trim panel removal set work fine.

What if the mirror heater or power fold doesn't work right away?

Check that your door harness is fully seated and the mirror connector is clean. If power and heat don't activate after a clean connection, the issue may be in the vehicle's door module rather than the mirror itself — test with a known working mirror first.
